An exciting opportunity currently exists for a Team Facilitator within our VSA@Home Service. This role will provide support to the VSA@Home Service, where you will liaise with colleagues and third party providers to ensure an effective and professional service is consistently delivered. This role will be facilitating & supporting in the growth within Aberdeenshire and applicants must possess a driving license & have access to a car,

Taking the lead in planning, monitoring and reviewing service delivery plans to meet individual needs and preferences, within budgetary and funding constraints. Administration of the Care Management System such as staffing rotas, daily management of staff diaries, updating service user observations, responding to alerts etc

Allocating work to teams and individuals to meet agreed operational objectives, ensuring staffing levels are maintained according to regulatory requirements

You will also work closely with the Service Manager in respect of current capacity and the ability to take on new services and/or replace services. You will be required to undertake home visits in the community to complete assessments for new Service Users, ensuring the allocation of appropriate staff to support their needs.

This will be a busy and fast-paced role where the ability to adapt to changing situations will be essential. You must possess the ability to think logically and will be an effective problem-solver who can respond calmly under pressure. You should be a sound communicator and willing to contribute positively to our fantastic team within VSA@Home.

The successful applicant must have previous care experience to be considered for this position.

Salary: £26,678

Location: Aberdeen, 38 Castle Street.

Hours: 39 per week (some evening and weekend working will be required)

Purpose of role:

To coordinate and contribute to the provision of quality services to meet the needs of service users, according to agreed VSA standards, policies and regulatory requirements, monitoring the effective use of staffing resources.

Main Duties/Responsibilities:

  • Promoting and utilizing ways of communicating effectively with individual staff members and service users on a wide range of matters including those of a complex or sensitive nature, progressing issues arising from the communication process where appropriate.
  • Overseeing and participating in the maintenance of accurate recording systems and reports (both manual and computerized) to meet agreed standards in line with administrative, regulatory and management purposes.
  • Monitoring the provision of a safe and protective environment by complying with health, safety and security procedures, including medication administration, and reporting accidents/incidents when appropriate.
  • Supporting service users in their daily living, according to their recognized and agreed needs, providing personal care where necessary, adhering to practice which promotes well-being, choice, giving people dignity, and treating them with respect.
  • Encouraging and supporting service users to develop skills and knowledge to live as independently as possible, if applicable, with reference to the risk assessment process.
  • Supporting service users in developing and maintaining social networks and relationships.
  • Performing the role of key worker for the service user.
  • Complying with and having a sound working knowledge of necessary legislative requirements as laid down through the Health & Social Care Standards and recognising the impact of following these standards on the overall provision of service.
  • Supporting service users in accessing and using services and facilities, assisting them to obtain advice in relation to financial matters if appropriate.

  • Allocating work to teams and individuals to meet agreed operational objectives, ensuring staffing levels are maintained according to regulatory requirements.
  • Conducting assessments of staff performance in accordance with VSA’s appraisal/supervision procedures, providing appropriate feedback and support to staff.
  • Contributing to the identification of the training and development needs of teams and individual staff members, taking appropriate action to implement plans to meet needs according to agreed procedures and guidelines.
  • Helping to maintain an effective working environment by contributing towards the management of interpersonal conflict in working relationships.
  • Assisting in the implementation of disciplinary and grievance procedures where appropriate, according to VSA’s policies and procedures.
  • Developing one’s knowledge and practice continuously through participation in training and development activities.
  • Carrying out other duties from time to time provided they are reasonable and within the post holder’s capabilities.
  • Have awareness and work within internal control systems in your area to facilitate improvement in the quality of service provided.
  • Basic IT Skills – Communication with Outlook is a large part of the role.
  • Maintain support plans ensuring these are up to date with the ever-changing needs of the service users.
  • Reception cover – Participate in shared reception cover as required between Monday to Sunday on a rota basis.

Qualifications/Training/Education:

Essential:

  • SCQF Level 6 in Social Services and Healthcare or other options as prescribed by SSSC.
  • Knowledge of SSSC Code of Practice.
  • Evidence of continued learning

Desirable:

  • SCQF Level 7 in Social Services and Healthcare + ability and willingness to achieve 15 academic credits in Supervision and / or Management at SCQF Level 7.
  • Knowledge of Health and Safety regulations.
  • Food Hygiene Certificate.
