Job Description
You will undertake assessment and care management and other Social Work responsibilities including liaison with health, private and voluntary agencies. Within care management arrangement, you will make assessments and negotiate packages of care in consultation with service users and carers to meet identified needs within defined budgetary limits. You will also monitor, review and assess individual care plans and respond to changing needs.
Requirements
You will hold a Diploma in Social Work or equivalent and have an up to date knowledge of Community Care legislation and practice. It is essential that you are able to communicate clearly, have good interpersonal skills and hold a driving licence.
Salary for the above post is inclusive of Distant Islands Allowance. Appointment to the post will be on the first point of scale.
Appointment to the post is for the duration of the maternity leave of the permanent postholder.
