The Home Care Service provides care in the home of Adults and the Elderly across the whole of East Lothian who require assistance and encouragement to carry out daily living tasks.
The Care Support Coordinator is required to respond sensitively and flexibly to a wide range of individual Service User situations and have the ability to work both individually and as part of a team. Care Support Coordinators work in partnership with a variety of Health and Social Care Professionals and report directly to the Senior Care Support Coordinator.
This is an excellent opportunity to work within a multi-disciplinary team within a Health and Social Care Partnership.
We are looking for enthusiastic and a dedicated individual who has a caring nature and excellent communication and IT skills, and has a knowledge in working in a Health and Social Care Partnership setting.
You will be required to work flexibly as part of a team on a 5 day rolling shift pattern. The team service operates between the house of 06:00 and 18:00 seven days a week including public holidays.
Due to the nature of the attached Post, flexibility is required at all times
As part of the Home Care Support Services Team, to assist vulnerable people to remain in their own homes through the support of Care Support Workers, in accordance with statutory obligations and Council policies and procedures.

A minimum of SVQ1/Standard Grade General level English/Maths or National English/Maths or Literacy/Numeracy or Intermediate 1 Communication/Numeracy and/or able to demonstrate equivalent knowledge, skills and competencies gained through relevant experience.
