East Lothian Health & Social Care Partnership serves a diverse, semi-rural community of around 100,000 people. Excellent road and rail links make us easily accessible from Fife, the Lothians and the Scottish Borders.

We are looking for a highly motivated and enthusiastic Community Care Worker (CCW) to join our Adult Social Work service, working across both the duty and assessment team.

Community Care Workers play a crucial role across Adult Services and are often the first point of response to referrals. You will be part of a wider team made up of Social Workers, Council Officers and Senior Practitioners.

This role covers a client group of adults 16+ with a range of support needs – Older Adults, Physical Disabilities, Acquired/Alcohol Brain Injuries, Mental Health, Substance Use & Autism. There is an element of hospital discharge work involved as well as crisis planning and a range of other interventions designed to provide a prompt response to service users in addition to longer term assessment, planning and intervention.

You will be able to manage challenging situations (with the appropriate support of your line manager) and apply a person centred approach to assessments with an understanding of Self Directed Support. It is key the post holder can work as an individual as well as part of a team and has excellent communication skills.  You will be experienced in developing effective working relationships with individuals, carers and external agencies as well as partnership colleagues.

Within defined boundaries, co-ordinate the provision of a range of services and equipment to meet the assessed needs of individuals requiring assistance, to enable them to maintain and maximise their independence, prevent admission to hospital or a care home, facilitate hospital discharge and to make choices.

Job Requirements

Relevant Qualifications
Educated to HNC or SQA Level 3 in social care or equivalent and be able to 
demonstrate equivalent knowledge, skills and competencies gained through relevant experience.

Driving Licence 
As the duties of this role require you to effectively travel to successfully undertake the full remit of the role, you must hold a current driving licence and have access to a vehicle.*
