Social Workers in the Care Home Assessment and Review team have a combined role in assessing people in hospital who may require care home placements, and for monitoring the quality of care within care homes. Social workers hold a mixed caseload of people in hospital and reviews for residents of care homes, and in addition act as link workers for the various care homes across the county.
The role is varied and ranges from supporting individuals through transitions such as moving to a care home, to being involved with investigations where provision of services falls short of the standards required.
The successful candidate should be able to communicate and empathise with people who may be in crisis, and have an understanding of the challenges for older people and their families who need social work and service provision.
To provide an effective service to service users and their families/carers, promoting a high standard of practice, in accordance with statutory obligations, policies and procedures.
