SENIOR HEALTH AND SOCIAL CARE SUPPORT WORKER, EAST CAITHNESS (BAND 3)

An exciting opportunity has arisen within the East Caithness Health and Social Care Team. We are looking for a motivated & enthusiastic individual who will be part of a multidisciplinary team working with patients in their own homes to provide holistic care. Your primary responsibility will be to provide support and assistance to patients to maximise and maintain their independence; working towards their own goals. This includes working to help support and facilitate timely discharge from hospital and working actively to reduce the potential of re-admission.

The post-holder will provide high standards of patient care in support of the team to ensure delivery of a high quality service to the local community.

A driving licence is essential for this post and applicants should have access to their own vehicle for work purposes.

This is a fixed term to October 2023 part-time post of 17.5 hours.
