Job Description
To assist the Community Mental Health Team to carry out procedures in relation to client community care needs assessments, ensuring current policies and initiatives set by Local and Central Government are applied.
Undertake home visits to contribute in the completion of needs assessments in relation to services being provided by Health and Social Care Department.
Requirements
Assist with the assessment for and provision of a social work service.
Advice / Support / Liaison in regards to service users and multidisciplinary colleagues.
Training and Development
Admin Duties and use of various IT systems.

Responsibilities
Contribute to assessments being prepared / undertaken.
Ensure visits are undertaken timely.
Provide advice and support to individuals, families and carers where appropriate.
Contribute to financial assessment and advice/assistance given in relation to claiming entitlements.
Contribute to the completion of SDS assessments.
Ensure clients have been given all documents in relations to care plans etc.
Provide accurate signed documentation to service providers in relation to a commissioned service.
Participate in meetings and joint visits with colleagues within Health and Social Care.
Assist Social work colleagues with duty system if and when required.
The Individual
Experience /Training in Local Government.
Knowledge and/or understanding of Community Care.
Educated to SVQ3, HNC or equivalent in relevant discipline with Higher grade or equivalent in English.
Knowledge and experience of Microsoft office packages, including Excel and Access.
Excellent verbal and written communication skills.
To be able to work under pressure & prioritise own workload.
Be able to exercise own initiative and make decisions.
You will be expected to travel efficiently and effectively between various work locations within Moray to meet the operational requirements of the Service. Due to the rural nature of Moray this is normally undertaken by use of a car.
Excellent numeracy skills with high level of accuracy.
Ability to deal with people in a calm, empathetic and confident manner.
Ability to work in a team.
Ability to be compassionate and have patience.
Ability to deal confidently with people who may present challenging behaviour.
Ability to work in an open plan office.
Ability to work in different offices dependent on service requirements.
Ability to work flexibly to meet demands of service.
