Perth and Kinross Health and Social Care Partnership
The Perth and Kinross Health and Social Care Partnership delivers joined up health and social care services across the Perth and Kinross area. The partnership consists of representatives from Perth & Kinross Council, NHS Tayside and other community partners.
We want to improve the quality of life experienced by the people who need the support of health and social care services. By working together, our aim is to ensure that people receive the seamless support they need when they need it, to live active, healthy and independent lives in their own homes (or a homely setting), close to their family, friends and/or local community for as long as possible. In Perth and Kinross, we recognise that people who are ill, vulnerable, or have disabilities often need support from numerous services to enable them to live independently and to prevent unnecessary stays in hospital or in residential care. Growing demand, means that, now more than ever, we need kind, compassionate and caring people to come and work with us in Social Care.
With the local community at the heart of what we do, we want to ensure that people receive the right care at the right time, adapt care to meet the needs of those that need it, build strong and compassionate communities, which offer support and companionship and make sure that people have the information and support that they need to manage their own health and wellbeing.
