Beechgrove House, Hillend Road, Perth

A great opportunity has arisen for an enthusiastic individual who has experience of organising and coordinating packages of support to vulnerable service users. 

You will lead and support a great team of staff who are skilled and experienced in delivering care and crisis intervention to vulnerable service users across localities in the Perth and Kinross Council area.

You will have excellent assessment and organisational skills and the ability to prioritise and support the delivery of a crisis response service to allow people to stay in their own home for longer.  

You will support covering any staff sickness absence with the support of one other weekend coordinator.

You will have the opportunity to demonstrate your ability to work effectively, support a team and demonstrate a commitment and an ability to contribute to service provision and ongoing developments.

The role will allow and require you to demonstrate your experience of multi-disciplinary working and be committed to working closely with colleagues from other disciplines. 

In this role you will be part of a developing service and support the "Locality Intermediate Care" model, supporting existing Integrated Care Teams and wrapped around Primary Care.

You will work closely with our Community Alarm team and Out of Hours team.

You will coordinate service centrally but will have the opportunity to work from local Health and Social Care Partnership bases across all Perth & Kinross geographical areas.

Hours of work will be Saturday and Sunday 07.00 - 15.00

The Individual

As a Perth & Kinross Council employee it is essential that you have excellent listening, oral and written communication skills as well as the ability to lead and work as part of a team 

You must also have the ability to maintain confidentiality, work co-operatively with other professionals, families and carers and be able to work on your own initiative.

This role will be ideal for you if you are enthusiastic, motivated and able to cope with demanding work. We expect you to be reliable and have a flexible approach together with a positive, responsible and caring attitude.

This will be a demanding post that will allow you to demonstrate your flexibility and showcase your proactive approach that can respond to the changing needs of individuals.

Experience of an electronic scheduling system would be advantageous.

It is essential that you have or be willing to obtain SVQ2/3 in Social Care, or equivalent Qualification.
