Role Purpose

An opportunity has arisen within the START team at Borders General Hospital for a Care Manager.

You will work as a member of an integrated START multidisciplinary team.

The primary task of the post holder will be to effectively and efficiently provide a high quality service to a range of adults and their carers as part of discharge planning.
This will involve providing advice and information, the need to work collaboratively with health colleagues and partners in the Third sector, good communication skills, and an ability to multitask and prioritise the competing demands of your case load. Undertake assessments and care management tasks in accordance with current legislation and council policies, procedures and standards.


Professional Social Work qualification
