This post is an excellent opportunity at a time of considerable development.  As a Social Work Assistant (Assessment and Review) within the Care at Home Team, you will have responsibility for undertaking community care assessments in line with Self-Directed Support and Carer legislation. The post will play a key role in the delivery of community services and will involve assessing service users’ needs and monitoring and reviewing care within agreed timescales.

You will work as part of a multidisciplinary team, under the supervision of Care at Home Team Leaders and provide support to deliver the Council’s statutory duties in relation to care management activity.

You should be able to demonstrate excellent organisational, IT and communication skills. Successful applicants will be required to be flexible in approach and have the ability to travel across the locality.

An HND or SVQ in Social Care, or equivalent, is desirable.
