An exciting opportunity has arisen for one full time equivalent Care Co-ordinator to join the newly created Assessment and Review Team. 

Successful applicants will be supported by a Team Manager and will work in collaboration with colleagues from other disciplines within the team which will comprise Occupational Therapists, Social Workers, rehab and technology staff.

The team will be responsible for the assessment for new community services, review of existing care at home services and oversight of unmet need in relation to Care at Home. 

The successful applicant will be solely employed to provide the necessary support to unpaid carers and will be responsible for working with unpaid carers to develop their Adult Care and Support Plan.

The post holder will be required to think creativity around how we support unpaid carers, whilst assessing for eligibility and formalised respite/support also offering a sound knowledge of what is available within communities and how this can be accessed to provide support and meet support needs

Successful applicants must have the ability to work as part of a team, respond to the demands of a busy service, be able to follow procedures, have excellent communication and good report writing skills.  They must have the ability to travel throughout West Lothian and hold a recognised qualification in social care.

The skills and knowledge applicants must have are: -

  • SVQ 3 or an HNC in Social Care or equivalent.  
  • It would be desirable for candidates to have relevant assessment and care management experience in an adults or older peoples social work practice team.

This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council.

This position requires registration with the SSSC. The successful candidate will have six months from commencement in this post to register with the SSSC.  If you are already registered with SSSC you will be required to add West Lothian Council to your account.
