Term: Glasgow - Fixed Term until 31 December 2022, with possibility of extension

Location: Drumchapel 2 x 17.5 hours posts or 1 full time post

Shettleston 1 x 17.5 hours post

As a Community Links Practitioner, you will be embedded both in a GP practice, and within the community that that practice serves. You will work with anyone, with any issue, who needs help – the only condition is registration at the GP practice in which you are based.

You will have the opportunity to create bespoke and innovative solutions in collaboration with the people you support, and to signpost ways for individuals to live well, on their own terms, in their own community. The support that you deliver is defined through close and non-judgemental relationships, and through a continual process of engagement with the local community.

You will make a difference to people’s lives by being there to help when they need you. There are no limits to the number of contacts you may have – if you can provide help, you can be there. You will contribute to easing pressures on the GP practice and will help to improve the wellbeing of the practice team. This role will allow you to become a valued member of the community and will contribute significantly to community wellbeing and resilience. You will help people to help themselves in surprising and creative ways.

In applying for this job, you must have experience of working with people who are experiencing complex social and emotional circumstances, and a strong understanding of the challenges faced by people living in areas of socio-economic deprivation in relation to living well.
