Job title: Senior Practitioner | Care at Home Services

Responsible to: Service Manager

Hours of work: Full-time permanent (36.25 hours per week)

Location: Field based working across all three localities of Aberdeen City (base office is Bridge of Don)

Salary: SCP 27-28 | £23,895 - £24,703

Main responsibilities:

Lead and participate in the provision of a high standard of person led care and support for service users in their own homes.

The Senior Practitioner will support the Lead Practitioner and Service Manager with the continued development and provision of our Care at Home service. The Senior Practitioner will carry out induction, mentoring and field based supervision of new staff and will support with on the job training.

Job activities

  • To understand the physical, emotional, cultural, and social needs of the service users, including a wide range of needs, illnesses and disabilities
  • To enable the provision of a wide range of domiciliary care including personal and continence care, managing medication, household tasks and meal preparation as agreed in individual care plans
  • An understanding of palliative and end of life care
  • An understanding of working with people who are elderly, people who have mental health problems and people who may be vulnerable in their home and their communities
  • Working to a person-centred and enabling approach
  • To observe and promote the service user’s choice, independence, dignity, privacy, fulfilment and other rights
  • To create and maintain good communication and develop effective working relationships with service users, families and multi-disciplinary teams
  • To actively support the Support Practitioners, through field based induction and by providing support and supervision in line with national and organisational guidelines and standards
  • Coordination and logistics of service provision
  • Development, implementation and review of risk managed and outcomes focused care plans with service users
  • To work pro-actively and responsively to ensure a continued high level of service provision and safe and appropriate risk management
  • Delivery and facilitation of training and accurate training record keeping
  • Deputise for the Service Manager when required
  • To encourage the independence and motivation of the service user and foster improved levels of self-sufficiency
  • To use manual handling equipment safely and correctly

Systems

  • Maintenance of records – Utilisation of our Care at Home systems (Care Planner System/PASS) to record all information in relation to Service User Management and Care Planning. Report writing, HSEQ checks, Risk Assessments and Risk Management Planning, etc
  • Day-to-day operation of service (appointments and meetings)

Person Specification

Essential:

  • A minimum qualification of N/SVQ3 in Adult Health and Social Care, or equivalent professional qualification (nursing etc) (or equivalent professional experience and a willingness to work towards SVQ3)
  • Excellent spoken and written English (must be capable of achieving CEFR/IELTS level B1 if required for a work permit application).
  • Literacy and numeracy at a level to maintain all the appropriate paperwork, write reports and support plans and give budgeting advice.
  • Able to use computer applications such as Word, databases
  • An understanding of social issues such as homelessness, mental health, substance misuse.
  • Driving licence.
  • Proven track record in a similar role
  • Experience of working with disadvantaged people.
