Community Link Worker West Lothian

Community Link Worker – Training provided Salary: £20,865 (pro rata for part time) £10.70 per hour.

We are a charitable company who have been delivering community based mental health services since 1983 and worked in the West Lothian region since 2014.

An opportunity has arisen to work in an innovative multidisciplinary service within the Community Wellbeing Hub in Livingston, as well as working on a sessional basis in G.P. practices across West Lothian.

We are looking to recruit a community link worker/s to deliver person centered individual support to people in contact with a G.P. practice team who may be experiencing complex social circumstances that are having a negative impact on their health and wellbeing.

You will support people to help them identify personal outcomes and priorities that they would like to address, and link them to local and national resources and services as required.
