An exciting opportunity has arisen for a registered social worker to join the Enhanced Community Dementia Team. 

The objective of the Enhanced Community Dementia Service is to provide specialist assessment, diagnosis and post diagnostic support. The service aims to be responsive to need using a person centred approach.

The successful candidate will have a strong sense of professional identity and an ability to work both independently and as part of a multi- disciplinary team.
