Job Description

Job Title: Support Plan Reviewer

Location: Various (based at either Duns or Galashiels)

Hours: Full Time

Contract: Temporary until 31/07/2025

Salary*: £13.02- £14.15 per hour/ £23,768.96 - £25,831.20 pro rata per annum

Training: All training is provided.

**A current driving licence and use of a vehicle is an essential requirement of this post.

Working closely with service users and their families, you will ensure the assessed needs of service users are recorded accurately and comprehensively to generate individual support plans, which will provide support staff with clear instruction as to the care that is required to be provided to service users.

You will build on the information contained in the Social Work assessment to assess and identify individual needs and risks by adopting a person centred and outcomes focused approach. You will ensure that all support plans are maintained as current and accurate, and are regularly reviewed as needs change. The support plan and accompanying paperwork must accurately reflect individual needs, choice and wishes.

To ensure that tasks identified in support plans are communicated/shared with Support Workers and that these tasks are clear, appropriate and agreed with the Service User or their representative.

To maintain close contact with support workers involved in direct care to service users, ensuring their feedback/concern in respect of service users, is acted upon. Through ongoing evaluation and review, you will be responsible for the recording of all relevant information specific to individual support needs and co-ordinating referrals for any area of unmet need as required.

To assess all areas of risk and develop appropriate risk management strategies, ensuring that this aligns with the information contained within the support plan.

Support plans will be produced within agreed time scales and in accordance with our statutory responsibilities and be authorised by the Assistant Home Care Manager/Regional Home Care Manager before being issued to support staff and service users.

It is your responsibility to ensure Assistant Home Care Managers/Regional Home Care Manager is consulted for advice and support in the event of any complex cases.

To be responsible for the planning and organising of service user review meetings in line with our statutory responsibilities, ensuring support workers feedback has been considered. This process must involve the service user and/or their representative. Any changes identified within the review process must then be updated within the support plan, and Social Work colleagues made aware of any changes.

Working closely with service users, a risk assessment will be undertaken to identify risks to service user/staff and must also include any associated environmental risk. Complete all agreed Risk Management Plans/Strategies, ensuring they are clear, appropriate, and relevant. These must be communicated to all members of the support team. Risk assessments must be reviewed at 6 monthly intervals, or as assessed needs change.
