Brokerage Officer 
Waverley Court 

Salary: £24,890 - £28,854 per year
Hours: 36 per week

• Undertaking relationship-based brokerage activities to ensure the effective use of available capacity, responding to service requests for the provision of care on a citywide basis.
• Supporting and sustaining effective communication and collaboration between Health and Social Care teams across adult social care and their managers.
• To continuously develop the business process to achieve a timely and customer focused, outcome-based approach to meeting the needs of people who require care to live as independently as possible in their own homes, in group homes (Supported Living), and in residential/nursing homes.
• Use of data management tools and systems to create and manage databases to:
• understand and share information on the supply and demand for Home Care (including Supported Living), and placements in residential/nursing homes.
• record actions, decisions and agreements, including placements. • provide accurate statistical, financial, provider and management information.
• Undertake the rationalisation process and actively seek out opportunities to make most efficient use of any Block purchased provision.
• Maintain and develop effective partnerships with local Providers, liaising with them by telephone, Microsoft Teams, email and letter on a day-to-day basis as required and appropriate to sustain confidence, trust, responsiveness and to negotiate with providers to maximise use of their available capacity and achieve best value and sustainability through optimisation of referral matched to the capacity available.
• Support Locality Teams to develop and sustain the value of Local Provider Forums to maximise the benefits to all partners ensuring that forums are well planned, and actions are recorded and implemented.
• Ensure that accurate case notes are created in the EHSCP Care Management Systems to concisely describe actions taken, negotiations and outcomes agreed.
• Contribute to the evaluation of the Brokerage Team to support practice and process improvements.
• Ensure that requests for brokerage are actioned efficiently, keeping to specific time frames and in accordance with Council policies and procedures and that all other teams involved in the matching process are appropriately communicated with as required, and that any relevant policies and procedures agreed are followed to ensure services are added to the required systems to support timely payment to providers for services delivered.

This post is considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by the City of Edinburgh Council.

Our salary range typically reflects the initial starting salary and annually increases until it reaches the top of the range.
