Job Description

This post is fixed term for 2 years from start date.

 

This is an exciting opportunity to be part of a newly formed Care at Home Review Team within The Clackmannanshire and Stirling Health and Social Care Partnership.  Where you will be part of a small team based in Clackmannanshire Council, Kilncraigs, Alloa.

 

To provide a statutory Social Work Service in accordance with the provisions of relevant legislation and national and local guidelines.

 

To assume case management responsibilities specific to the Service, carrying out assessment of service users needs and risks, following Service procedures and delivering direct interventions to individuals and small groups as part of a programme of interventions. The role will also involve participating in structured consultation/supervision sessions and representing the Social Work Service in multi-disciplinary forums. This will all be in accordance with Council or hosted stakeholders arrangements, through a Service Level Agreement or Partnership arrangement.

This post is considered Regulated Work with Vulnerable Adults under the Protection of Vulnerable Groups (Scotland) Act 2007. You will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by Clackmannanshire Council. Further guidance will follow in your conditional offer of employment on how to apply. Please note you will be required to pay for the cost of this application, if your salary is over £30,000 per annum, which will be deducted from your first salary.
