The post holder will be based within the Community Mental Health Team for Older Adults (CMHTOA) in Huts 3 & 4, Falkirk Community Hospital.

As part of the Older Adult Community Mental Health Team the post holder’s role will include holding a caseload of people experiencing mental health issues and also supporting the Alzheimer’s Scotland 8 Pillar Dementia Model for people who have received a diagnosis of later stage dementia to access community supports, care and treatment on an ongoing basis and ensure effective intervention across health and social care with the overarching aim being to support the person to remain at home for as long as possible. 

We are seeking a dynamic and forward thinking Clinical Care Support Worker. Experience working in older adult is essential. Experience working within a community mental health setting would be advantageous.

The post holder will be the named, skilled practitioner operating at the Enhanced Level of the Promoting Excellence Framework who will provide a person centred approach for one year following diagnosis.

The post holder will work across the Falkirk Health and Social Care Partnership to provide a comprehensive, multi-disciplinary community mental health service to patients and carers in partnership with other agencies.

Ability and willingness to travel throughout the area of service delivery is essential for this post.
