An experienced, motivated, and ambitious practitioner to lead and support service delivery and provide high quality support and supervision to team members in our Home First Team.
The Home First Team ethos focuses on the following: working within a healthcare system where we have integrated health and social care, with a clear focus on prevention, anticipation and supported self management. Ensuring the patient is at the centre of all decisions.
The team work in Partnership with health towards ensuring that people get back into their home or community environment as soon as appropriate, with minimal risk of hospital re-admission.
Falkirk HSCP have 2 intermediate care sites (slow stream and rehabilitation), and the Home First team are vital to supporting service users within their intermediate care journey.
The Home First Service is undergoing an exciting period of change and development.
The Team Manager will work as part of the Falkirk Health and Social Care Partnership Management Team to deliver services in line with the vision, values, legislation, policies and procedures for Health and Social Care services, both locally and nationally.
Applicants must be trained in relevant risk assessment tools, have significant experience in adult social work practice along with experience of supervising and managing workers.
