Job Advert

This is an exciting time to join the Falkirk Health and Social Care Partnership and we are now looking to appoint 2 x Homefirst practitioners to our Hospital team.  You will be a motivated worker who is both enthusiastic and committed to improving the lives of our service users, who practices in a way that promotes better outcomes and focuses on strengths, rather than deficits. You will be primarily based at the acute hospital site at Forth Valley Royal. This is full time hours, 8am to 4pm. Homefirst is a 7 day service with workers working 5 days over 7, which includes a rota for weekend work.

Homefirst practitioners work in a high paced reactive and responsive environment as part of the Health and Social Care Partnership. This role includes partnership working to review and support discharge planning for service users. The team facilitate timely discharges to home, or to intermediate care, working in partnership with the integrated health team and to achieve the best outcome for service users

We have recently recruited a senior locality leadership team that is committed to ongoing professional practice development and career progression. We are fully committed to strengthening professional practice. We are currently undergoing service-wide transformation that will not only improve outcomes for our service users, their carers, families and communities, but will also see the development of a stronger Social Work service within the partnership.

Working with local communities and a wide range of partners, our services continue to develop to support our ambition to help adults live safe, healthy and independent lives by offering enhanced preventative, pro-active and responsive community services.

You will join our multi-skilled team; working closely with colleagues from within the health and social care partnership, in addition to a wide range of external partners.

In return, we will offer you on-going professional development opportunities, a supportive environment with regular professional supervision and access to training. We are currently developing a clearer structure for career progression opportunities. You will also receive the support of a committed senior leadership team, which sees the true value of Social Work, where despite the pressures of budget savings and increased demand for services, we are committed to strengthening the role.

An SVQ in Health and Social Care, or equivalent, is desirable and experience of working within a social work or social care setting is required.

Candidates with experience working in a Social Care Officer or Social Work Assistant capacity may find this a rewarding career move.

If you are the successful candidate, you will be required to gain/maintain PVG scheme membership.
