The successful candidate will provide vision, leadership and operational oversight for a wide portfolio of older people and primary care services including Older People and Physical Disability and Mental Health services, Occupational Therapy Services, District Nursing Services, Hospital Social Work Services, Community Rehabilitation Services, Intermediate Care, Technology Enabled Care, Complex Care and Supported Living Services, Primary Care including General Medical Services, and Health Improvement Services and Equalities.

They will be responsible for ensuring the delivery of high-quality, person-centred services that meet the strategic objectives of the HSCP and comply with national standards and statutory responsibilities.

This role requires a dynamic and experienced leader who can drive forward transformational change, develop and implement commissioning and workforce strategies, and lead service redesign and integration.
