An exciting opportunity has arisen to join Glasgow City HSCP's Care Services Occupational Therapy team at a time of significant development and change. As we review and reshape how services are delivered across the city, this role offers a genuine opportunity to influence future models of care, contribute to service improvement and help shape the direction of one of Scotland's largest care services.

Working within an Occupational Therapy-led assessment and reablement service, you will play a central role in supporting people to remain independent in their own homes, preventing unnecessary hospital admissions, and supporting timely discharge from hospital. You will work closely with colleagues across health and social care to deliver person-centred assessment, intervention, and review, helping people achieve outcomes that matter to them.

This is a service operating at significant scale:
• Supporting more than 4,800 people across Glasgow through Home Care services.
• Delivering over 4.2 million home care visits each year.
• Supporting more than 8,600 hospital discharge referrals annually.
• Providing reablement support to over 3,300 people each year.

The Occupational Therapy service sits at the heart of Glasgow's Maximising Independence approach. Every new referral is screened for reablement potential, ensuring people receive the right support at the right time. The impact of this work is clear, with almost half of those completing a reablement programme requiring no ongoing home care support. This not only improves outcomes for individuals but also helps people maintain their independence and remain connected to their communities.

As an Occupational Therapist, you will undertake functional assessments, provide equipment, and minor adaptations, support goal setting and deliver tailored interventions designed to maximise independence. You will manage a varied caseload and work alongside Social Care Workers, Home Carers, Team Leaders, and wider multidisciplinary teams.

Applicants must possess a Degree / Diploma in Occupational Therapy.

Membership of the Protection of Vulnerable Groups (PVG) Scheme is essential on taking up appointment. Please note successful candidates for the above role will be required to pay for their PVG Membership/or Scheme Checks.

Registration to the Health and Care Professions Council (HCPC) is an essential requirement for this role.
