Job Description

The Glasgow Health and Social Care Partnership (GHSCP) is on a journey to radically change the way in which Social Work delivers support and services.

An exciting opportunity has arisen for a Social Care Worker within the Health and Social Care Connect Service. This service is a single point of access for Social Work Services within Glasgow HSCP. Changing the way we deliver services using an asset-based approach and have effective interface with partners and the 3rd sector. The service will span across Adults and Older People’s Services, Children and Families and Homelessness, working in a multi-disciplinary way.

We are seeking energetic, talented, resilient and committed social care workers to prevent long term involvement with statutory services where possible and provide excellent services to those who require it.

You will have sound knowledge of assessment and care/ case management; have practised under the relevant legislation; and work in a person-centred manner which will be fully embedded in your practice. You will be confident in managing risk and looking for innovative ways to promote independence and link people into the local community.

Applications will be considered from people with experience from the aforementioned care groups, including occupational therapy assistant experience. HNC/ SVQ3 in Social Care or equivalent qualification would be advantageous.

Membership of the Protection of Vulnerable Groups Scheme will be essential on taking up appointment. Please note successful candidates for the above position will be required to pay for their PVG Membership/or Scheme Checks.
