Role Purpose

This is opportunity for an enthusiastic, energetic and motivated Assistant Team Leader with experience in Adult Social Care to join our recently formed Discharge Team based in the Borders General Hospital.

We are a multi-disciplinary team with both Social Work and NHS working in partnership with service users, carers, family members and external agencies to facilitate discharges from an acute hospital and Intermediate care settings.

Essential

Recognised professional qualification in social work, nursing, occupational therapy. A current driving licence and use of a vehicle is an essential requirement of this post.
