Role Purpose

We have an opportunity for an enthusiastic, energetic and motivated Social Worker/Care Manager with experience in Adult Social Care to join our recently formed Discharge Team based in the Borders General Hospital (BGH). You'll work in a fast paced environment as part of a friendly and supportive multi-disciplinary team. Every day will bring new challenges but also the opportunity to make a real difference to the lives of patients in BGH and their families. Working closely with NHS clinicians, care providers, patients and their families you will assess and facilitate arrangements for patients leaving an acute hospital setting.

Essential

Professional Social Work qualification.

Registration with the Scottish Social Services Council.

Registered Nurse registered with Nursing and Midwifery Council.
