Job Description

What is the role?

To provide a Community Care Assessment and Care Management Service to individuals within the following client groups: older people; mental health; learning disabled; physical disability and addiction.

An exciting opportunity has arisen to work within the Hospital Social Work Team, based at University Hospital Ayr.  We are seeking to recruit individuals with drive, energy, and a commitment to provide an effective and efficient social work service to adults and carers across South Ayrshire enabling them to Grow Well, Live Well and Age Well.

Although based at University Hospital Ayr, the successful candidates will work with patients across all hospital sites to support discharge.  Building on the existing positive relationships with our NHS Ayrshire & Arran colleagues the successful candidates will form a vital part of the multi-disciplinary discharge team. 

These posts offer an exciting opportunity to join a forward-looking Health and Social Care Partnership and be part of a team looking to deliver an efficient, effective, well organised and responsive service.

The Grade of these posts are Grade 3 / Level 10; the hourly rate is from £21.75 to £22.88 and the annual salary is from £39,694 - £41,756. 

The available posts are as follows:

Post A - 35 hours Perm x 2

Post B - 35 hours Temp

Applicants whose qualification award is pending within the next 6 months, are welcome to apply.  If you are recommended for the post the initial placement will be at Level 8, until your Qualification is awarded.

This post is  considered to be Regulated Work with protected adults and children as specified in the Protection of Vulnerable Groups (Scotland) (PVG) Act 2007. Accordingly the preferred candidate will be required to become a member of the PVG Scheme, or undergo a PVG Scheme Update if already a Scheme Member, prior to any formal offer of employment being made.
