Job Description
An exciting opportunity has arisen for a care manager to join us! We welcome applications from a Social Work, Nursing and AHP background. The post holder will be part of an Hospital and Intermediate Care Team located within the Stirling Health and Social Care Partnership. This post is 35 hours per week and the post holder will be line managed by the Team Manager of the Hospital and Intermediate Care Team. You will work within an outcomes based framework to carry out individual assessments, and to plan, manage and review care.
