Job Description
Based at Stracathro Hospital, Brechin.
This job is workstyle 3 - Flexi.
You will have the opportunity to benefit from our flexible and agile working practices. This means you will work from a variety of locations, working from council buildings, working in service user’s locations and site visits and you can work from home to best suit your work, meetings and collaboration.
The role of the support worker is to continue post diagnostic support once this has been established and planned by the link worker, and encourage community engagement by supporting service users to attend meeting centres and community activities. Support workers also complete written wellbeing plans and getting to know me documents as well as closing cases and preparing discharge letters. They ensure that all 5 pillars of PDDS support have been met.
