Job Description
To work as a role model and support the delivery of competent and confident professional practice within Care at Home.
To plan, support and co-ordinate Care at Home practice relating to complex needs for adults within Moray in line with local and national policy and legislation and to take responsibility for supporting with the management of complex situations.
Requirements
The effective management of caseloads, the delivery of cost-effective interventions and the achievements of all relevant statutory requirements and Department policies, standards and guidelines. To introduce and support the development of evidence based practice.
Provide support, mentoring and coaching to Care at Home staff and demonstrate best practice in a professional capacity and promote a learning culture, particularly in relation to risk assessment & management and understanding complex needs.
To be responsible for practice audit within teams, working together with other Care at Home Management and the Quality Performance Team.
Manage, Plan and Co-ordinate set standards and monitor quality of complete needs in Moray in line with local and national policy and legislation.
The Individual
Recent experience of working in a Health/Social Care setting
Excellent knowledge of legislation, policies and practices associated with Care at Home and community care
Experience of successfully managing a varied and complex caseload
Experience of supporting/managing staff
HNC/SVQ3 in Social Care
Registered Nurse or other applicable professional qualification
Evidence of continuing professional development
Ability to negotiate and liaise effectively
Effective communication skills – oral and written
Effective use of IT skills and experience of using IT packages
Ability to work flexibly to meet demands of service
Excellent written and verbal communication skills
Ability to present complex information to colleagues
Ability to work independently
Ability to support and mentor staff
Knowledge of SSSC codes of practice and evidence of understanding/application in relevant work setting
Tact diplomacy and patience
Driving licence. You will be expected to travel efficiently and effectively between various work locations within Moray to meet the operational requirements of the Service. Due to the rural nature of Moray this is normally undertaken by use of a car
Organised and able to manage competing priorities
Ability to work in a an open plan/shared office
Flexibility to work in different areas when required
