As part of the West Lothian Health and Social Care Partnership (HSCP), the Team provides an assessment and care management service to adults under 65 with a Physical Disability.
The successful candidates will be based within a well-established and developing team. This is an interesting and challenging opportunity for an enthusiastic person to work with individuals with a range of complex needs who require support to maintain their independence. Candidates should ideally have experience of working with this client group.
The successful candidates will have an ability to work as part of a team, respond to demands of a busy service, be able to follow procedure, have excellent communication and report writing skills. These tasks will be carried out in partnership with service users, family members, social work colleagues and other professionals and agencies.
The posts require a good understanding of self-directed support and outcome focused assessments. You will also undertake Duty work and Adult Support & Protection investigations in accordance with service standards.
A professional Social Work qualification is essential.
This positions require registration with the SSSC. The successful candidates will have six months from commencement in this post to register with the SSSC. If you are already registered with SSSC you will be required to add West Lothian Council to your account.
These posts are considered Regulated Work with Vulnerable Children and/or Protected Adults, under the Protection of Vulnerable Groups (Scotland) Act 2007. Preferred candidates will be required to join the PVG Scheme or undergo a PVG Scheme update check prior to a formal offer of employment being made by West Lothian Council.
